Thank you for your interest in Mattscasbah as a prospective contributor to your organization. To help us evaluate all requests fairly and consistently, we ask that solicitations for charitable gifts be submitted using the form below.

Please take a moment to review our donation policy, as we can only honor requests that meet the following guidelines.

• Please note that because we are a Florida-based company, we will give priority to local organizations. Donations must be picked up at our Business, located at 801 East New Haven Avenue, Melbourne, FL 32901. Shipping is not available for charitable donations.
• We accept requests from non-profit organizations only and will not grant donations to for-profit organizations or businesses. As part of the on-line request form, you will be asked to provide a non-profit tax ID number.
• Requests for donations will not be accepted by phone.
• We ask that all donation requests be submitted prior to the date they will be needed. This will allow us ample opportunity to review the information and correspond as necessary.
• We request a letter of acknowledgment following the donation for our own records.
• Please fill out the entire form. We will only consider applications that are filled out entirely.
